In Microsoft Internet Explorer, go to:
Tools (top menu) → Internet Options → click on Security tab → click on Custom Level → scroll down to the bottom until you see Scripting → Active Scripting → Choose "Enable."
After you have done this, refresh the page and you will see the login form.
